Health and fitness screenshots work when people can picture the progress: a finished workout, a sleep score, a streak or a calm session. Put that outcome in the first screenshot with one short benefit, then show the routine that produces it, such as plans, tracking, reminders and history.

Rings, streaks and charts persuade only when they are readable at product-page size. Crop in on the number that matters and keep the caption to a few words.
Label what needs a subscription.

Keep health data fictional and claims modest.

App Store Connect accepts exact pixel sizes only. Apple currently lists at least one iPhone with Dynamic Island (medium display) screenshot for iPhone apps, plus a 13-inch iPad screenshot if the app supports iPadOS. Check the selected submission slot and the display-scaling notes in App Store Connect. The medium Dynamic Island group accepts 1179 × 2556 or 1206 × 2622, and the 13-inch iPad group accepts 2064 × 2752 or 2048 × 2732.
